How does it work?

1. Fill out the contact form to the left with as many details as you have regarding your project. Someone from the SurveyAct team will contact you to get started.
2. Our designers will create a theme based on your specifications, and will tweak it until you are satisfied.
3. Once the theme is created, you can log in to your account and use it as many times as you like!

How much does it cost?

A custom theme design is $99/theme.

Fine Print

The survey elements (ie, questions, progress bar, buttons) cannot be moved or resized as part of your custom theme. One set of revisions is included in the base price; additional revisions will be extra. The theme will go into the account of only one user. Payment is due in advance. Turnaround time is 3-5 business days.
