Adding Special Question Types
SECTION HEADER
Use this question type if you would like to enter text for survey respondents to read, such as an introduction.
To Create this Question
- Enter the text heading in the ‘Description’ box. To go to the next line, press shift+enter.
- Choose your optional settings from the right hand side of the question editor
- To add the question to your survey, click Save
- Change Question Size & Placement
DEMOGRAPHIC FORM
Use this question type if you would like respondents to choose their demographic information from a list of predetermined options.
To Create this Question
- Enter the question heading in the ‘Question Text’ box. To go to the next line, press shift+enter.
- The default demographic options are Gender, Age, Household Income, Race, Standard Education, Occupation, Employment, Marital Status, and Residence. You can change the options by typing over the existing texts. You can also make some invisible, and choose whether each field should be required or not. Standard demographic responses will automatically be included in your survey.
- Choose your optional settings from the right hand side of the question editor
- To add the question to your survey, click Save
- Show or Hide individual fields
- Require Answer to Question
- Change Question Size & Placement
DATE & TIME ENTRY
Use this question type if you would like respondents to choose a specific date and/or time as their response.
To Create this Question
- Enter the question heading in the ‘Question Text’ box. To go to the next line, press shift+enter.
- From the drop down menu, choose date only, time only, or both date and time. If you chose a date option, you can then choose to change the format using the drop down menu on the right.
- In the box called ‘input choices,’ type or paste your labels for each line one label per line. The number of labels will indicate the number of text boxes.
- Choose your optional settings from the right hand side of the question editor
- To add the question to your survey, click Save
- Change the Sort Type
- Require Answer to Question
- Change Question Size & Placement
IMAGE
Use this question type to upload an image for survey respondents to view.
To Create this Question
- Enter an image description or caption in the ‘description’ box.
- Click the ‘Upload Image‘ box to browse for images on your computer, then click upload. To clear the image, click the x in the top right hand corner.
- Choose your optional settings from the right hand side of the question editor
- To add the question to your survey, click Save
- Change Question Size & Placement
FORM QUESTIONS
Use this question to create a non-numbered group of questions that can stand on their own or link to multiple choice questions.
To Create this Question
- Enter the question heading in the ‘Question Text’ box. To go to the next line, press shift+enter.
- Choose your form from the list of previously created forms, or click the “forms” link below to create a new form.
- Choose your optional settings from the right hand side of the question editor
- To add the question to your survey, click Save
- Change Question Size & Placement
EMAIL SUBSCRIPTION
Use this question for respondents to sign up to be a part of a contact group that regularly receives your surveys.
To Create this Question
- Enter the question heading in the ‘Question Text’ box. To go to the next line, press shift+enter. Choose the e-mail group that you would like respondents who fill out the form to be subscribed to
- The default field labels are e-mail address, first name, last name. The responses to these fields will be automatically saved in your e-mail groups.
- Choose your optional settings from the right hand side of the question editor
- To add the question to your survey, click Save.

It is a good idea to include a message in the question box explaining to respondents that by providing you with their contact information, they are agreeing to receive future surveys from you.
To change the formatting of any question, use the Rich Text Editor.
