How to Use Forms in a Survey

There are two possible ways to link forms to your surveys.

  1. As a Form Question:  Inserting your form into a Form Question type will display your form as a question in the survey.
  2. As a Subform: You use a form as a subform for single and multiple response multiple choice questions. The subform will only appear once the user selects a response to the question. This can be useful if you want to collect secondary data that corresponds to a specific response.

How to Use a Form in a Form Question

  1. In the survey editor, click Add Question.
  2. Choose Form Question from the Special Question Type category.
  3. Enter the question heading in the ‘Question Text’ box.
  4. Choose your form from the list of previously created forms, or click the “forms” link below to create a new form. 
  5. To add the question to your survey, click Save.

How to Use a Form as a Subform

  1. In the survey editor, click Add Question.
  2. Add a question with subform* capabilities.
  3. Enter the question heading in the ‘Question Text’ box.
  4. Enter your response choices.
  5. On the right hand side, choose your form from the Subform drop down menu, or click the “forms” link below to create a new form. 
  6. To add the question to your survey, click Save.

*The following questions have subform capabilities:

  • Multiple Choice-Radio Button
  • Multiple Choice-Checkbox
  • Image Choice (Single Responses)
  • Image Choice (Multiple Responses)

Forms Vs. Subforms in Reports

Depending on whether you use your form as a form question or a subform, your reports will look a little different. Form question responses will appear in your reports the same way any other question would. Subforms will appear as ‘comments’ below the question they are linked to, and the combined results can be viewed by clicking on the View link that appears next to the question.
