How to Create a Form

  1. From the sidebar, choose Forms, then New Form.
  2. Give your form a name that will be easy for you to remember, so you can link it to your surveys.
  3. If desired, you can change the form size by entering a new number in the width or height box or by dragging the bottom or sides.
  4. Drag and drop one of the 5 form elements to your form. Make sure to align the element exactly where you want it to appear. Click Edit to customize your form element.
    1. Text Line- Enter your text.
    2. Input Box- Enter a name (optional- will not be displayed to respondents), and the size of the input box
    3. Check Box- Enter a name (optional), a label, and the response choices
    4. Radio Button - Enter a name (optional), a label, and the response choices
    5. Combo Box- Enter a name (optional), a label, and the response choices. You can check the response next to Hide First Option and the first response choice will not appear until the drop down arrow is clicked.
  5. When you are finished editing an element, click Done to apply changes. Continue adding elements as desired.
  6. When you are finished creating your form, you can preview it by clicking on the magnifying glass icon at the top right.
  7. Click Save and Continue.

 

TIP:  You can use the Form tool do add an ‘other’ or ‘comment’ response to questions that don’t have that option included, such as image choice questions or a demographic form.
